Who We Are

Advance Care Planning Volunteer Facilitators work under the supervision of Presbyterian's Advance Care Planning Department to provide support to patients and the community in all advance care planning functions.

What We Do

Our trained volunteer facilitators provide advance care planning support and education to patients, families and the community. This support may be provided through 1-on-1 visits, group settings, or presentations. It depends on the preference of the volunteer. Examples of discussion items include:

  • Understanding the importance of an advance healthcare directive
  • Selecting a Healthcare Decision Maker
  • Using individual beliefs, goals and values to complete an advance directive
  • Sharing wishes and values with family members
  • Providing copies of advance directives to healthcare providers

Also providing support to individual advance care planning projects such as National Healthcare Decisions Day, other community events, and follow-up phone calls to patients.
