Our trained volunteer facilitators provide advance care planning support and education to patients, families and the community. This support may be provided through 1-on-1 visits, group settings, or presentations. It depends on the preference of the volunteer. Examples of discussion items include:
- Understanding the importance of an advance healthcare directive
- Selecting a Healthcare Decision Maker
- Using individual beliefs, goals and values to complete an advance directive
- Sharing wishes and values with family members
- Providing copies of advance directives to healthcare providers
Also providing support to individual advance care planning projects such as National Healthcare Decisions Day, other community events, and follow-up phone calls to patients.
